Feature Proposal: A form that creates a new topic using a template should be able to upload an attachment

Motivation

It is very useful to be able to do single-pass topic creation from form entry (using the "save" action). If the new topic needs to have a file attached, this can't currently be done in the initial topic creation.

Description and Documentation

It should be possible to have a form for the creation of a new topic, that contains an <input> element of type "file", and a corresponding %META:FILEATTACHMENT% element in the template topic that the form uses, and have the file that is uploaded by the form be automatically turned into an attachment of the new topic with a filename specified in the %META:FILEATTACHMENT% name= property, and content determined by the form upload.
