LDAP Auth Occasionally Fails For User

We are using the LdapContrib extension for user authentication. We have it set up so new users are automatically registered when the log in the first time. It works great. However, every now and then a user (seemingly a random user) will NOT be able to authenticate even though they use the wiki everyday. They usually can not log in for at least 24 hours or longer! The event log shows the following:

Notice that the second field says "guest". All other log ins will have the user's username there instead. Also, there LDAP config looks normal to me. It's frustrating because I'm trying to push corporate wiki adoption but this is making it difficult!
