Change summary:

The switch-to-WYSIWYG button was created by javascript, and the javascript code defined where it got displayed. This was not always at the position a template designed would place it.

To allow more control over the layout of the edit screen, the WYSIWYG button is now defined in an edit template. If the button is not present (for instance in older templates), the button is created by javascript as it was before.
